Doctor(医師:kusu-shi) is a government position under the Ritsuryo system(律令制) in ancient Japan. They were studied at Ten-Yaku Dormitory(典薬寮), and only who passed the graduation and appointment examination was appointed to the doctor.
The reason for assigning the snake as a doctor is an allusion to the fact that the snake symbolizes the Greek god of the medical arts, Asclepius, and his daughter, Hygieia, who presided over the maintenance of health and hygiene.
